Copy of Ohio Individual Attendee Data Collection – Sample Spreadsheet for Data Entry

A link is attached with the Ohio Individual Attendee Spreadsheet template. This spreadsheet has been created for evaluation purposes, and is intended to help 21st CCLC grantees plan ahead for data reporting associated with 2015-16 programming (reporting of 2015-16 data will take place in late summer or fall 2016). The template has three tabs; the first includes data-entry fields, the second tab includes instructions, and the third tab shows values for the drop down fields on tab 1. Please note that final reporting requirements associated with 2015-16 programming may be slightly different than what is presented in this template (notification of any changes will be sent to 21st CCLC grant directors).

NOTE: Reporting 2015-16 (summer 2015, fall 2015, and spring 2016) attendee data is a requirement for first and second year grantees (FY15 and FY 16) only.
